We came in mid-modernization: architecture in flux, service boundaries getting redrawn, defect density high, and security and functional issues bleeding into each other. QA had to hold the line on release safety while engineering rebuilt the foundations underneath.

We ran QA governance through a high-change modernization phase: full defect lifecycle ownership (reproduction, triage, fix verification, regression follow-up), security-remediation closure validation across auth, session and transaction surfaces, weekly release-readiness gates, and cross-team supervision to keep quality consistent across distributed contributors.

What We Learned

Refactor-heavy environments need QA acting as a continuous signal, not a checkpoint. Tying defect closure quality to weekly release gates kept regressions from compounding while architecture moved. The discipline isn't in the tools, it's in the closure criteria.
